Improper Food Labeling, Storage, and Cleaning Practices

Summary

The facility failed to follow proper sanitation and food handling practices in the kitchen and nourishment rooms. During a kitchen tour on 5/18/2026, surveyors observed one open bag of thickener powder in the dry storage room, two open boxes of frozen beef patties in the freezer, and one box of pasteurized eggs in the refrigerator, all without labels or date markings. The same tour also found one expired carton of fat-free chocolate milk, two expired cartons of whole milk, and nine half-empty cartons of whole milk stored in separate crates inside the refrigerator. On follow-up observations, expired whole milk cartons continued to be found in the refrigerator on 5/19/2026 and 5/20/2026, including cartons dated 5/13/26, 5/12/26, and 5/13/26. During the second kitchen tour on 5/19/2026, food residue and debris were observed stuck on and above the safety guard of the mixer, and the microwave hood was filled with dried food splatter and residue. The same residue and debris on and above the mixer safety guard were again observed on 5/20/2026. A tour of two nourishment rooms on 5/20/2026 revealed residue build-up and unclean surfaces inside the refrigerators in both rooms. Staff interviews showed inconsistent understanding of who was responsible for receiving and stocking food deliveries, labeling and discarding opened food items, removing expired or half-empty milk cartons, cleaning kitchen equipment, cleaning nourishment room refrigerators, and documenting temperatures on nourishment room logs. The facility’s Food Receipt and Storage policy required open food items to be covered, labeled, and dated, and the Cleaning Schedules policy required the FNS Manager to supervise adherence to the cleaning schedule and inspect the kitchen weekly for cleanliness.
