Infection Control Failures With Contaminated Equipment and Wound Care

Summary

The facility failed to ensure resident care equipment was cleaned and disinfected between uses. During observation, Sit-to-Stand assistive Device #7 was seen on the hall with a maroon-colored substance splattered on the floor/platform portion of the device. On a later observation with the DON, the same device was again observed with the same maroon-colored substance still present. The DON stated that the equipment was expected to be cleaned before and after each use with purple disinfectant wipes and that Device #7 needed to be cleaned. The facility policy on Transmission Based Precautions stated that resident care equipment used for multiple residents must be cleaned and disinfected before use on another resident. The facility also failed to ensure hand hygiene and PPE use during wound care for Resident #66. Resident #66 was admitted with diagnoses including diabetes mellitus and atherosclerosis of the left leg with ulceration of the foot. Physician orders included enhanced barrier precautions related to a wound on the left foot/toes and wound care to the left great toe every evening shift. During observation, an LPN entered the resident’s room without performing hand hygiene and without donning gloves or a gown, removed the sock from the resident’s left foot, and exposed the left big toe dressing. The LPN then removed the soiled dressing without hand hygiene or PPE, and the dressing was saturated with a dark substance. After leaving the room and returning, the LPN went to the wound treatment cart and touched supplies with bare hands, including loose 4 x 4 gauze, and returned the packet to the cart. The LPN stated she was unable to locate wound cleanser and then went to another supply area to retrieve it. The LPN did not perform hand hygiene before exiting the room. The LPN later stated she should have completed hand hygiene with soap and water and should have worn a gown and gloves before providing wound care. The DON stated that hand hygiene should have been completed before and after each patient contact and before and after wound care, and that enhanced barrier precautions including gown and gloves were to be worn anytime wound care was provided.
