Summary

The facility failed to ensure the routine provision of ordered respiratory services for a resident with COPD and a pulmonary mycobacterial infection who was ordered oxygen at 3 liters continuously by nasal cannula. The physician also ordered the resident's humidification bottle and oxygen tubing to be changed weekly on Wednesdays on the night shift. Review of the resident's MAR/TAR from 02/04/26 through 03/05/26 showed no documentation that the humidification bottle and tubing were changed on 02/11/26 or 02/25/26 as ordered. During multiple observations, the resident was seen in bed wearing oxygen running at 3 liters per minute continuously, but there was nothing on the tubing to show when it had last been changed. There was also no humidification bottle attached to the oxygen compressor. RN1 confirmed the tubing had no date indicating when it was last changed, confirmed there was no humidification bottle attached as ordered, and confirmed there was nothing in the record showing the tubing had been changed. The DON stated that oxygen tubing and equipment were expected to be changed at least weekly and documented on the MAR/TAR, and confirmed the resident's oxygen should have been humidified.
