Delayed Provider Notification for Resident's Right Ankle Pain

Summary

The facility failed to timely notify the provider when a resident reported right ankle pain to staff. The resident had diagnoses including age-related osteoporosis and a left lower leg amputation, and the clinical record showed moderate cognitive impairment, right and left lower extremity impairments, and dependence for assistance to stand and transfer from bed to chair. On 10/24/25, therapy documented that the resident’s right ankle appeared slightly more everted than usual, with no swelling or redness noted, and nursing was made aware. Later that same day, occupational therapy documented 5/10 right ankle pain during attempted PROM, stopped mobility immediately because of the pain, and notified nursing of both the pain and a small cut on the right middle toe. An administration note documented that Tylenol was given for right ankle pain, and the next morning a nurse documented that the resident complained of right ankle pain and that the supervisor and MD were notified, with the provider notified 8 hours after the change in condition was reported to staff. Later documentation showed the resident continued to report right ankle pain, swelling was noted, and a STAT x-ray was ordered, which revealed an acute fracture in the distal shaft of the right fibula. Interviews with therapy and nursing staff confirmed that therapy alerted nursing to the ankle pain during the afternoon session, that the nurse gave Tylenol, and that the nurse did not assess the ankle pain or notify the provider at that time.
