Summary

The facility failed to ensure that residents were free from significant medication errors when a nurse administered the wrong resident’s medications to a resident with diabetes mellitus, chronic kidney disease, lymphedema, asthma, neuropathy, and hypertension. The resident’s care plan directed that medications be administered per physician orders, and the physician’s order required Daily-Vite, Fluticasone Propionate nasal spray, and Eliquis at 8:30 AM daily. Instead, the resident received another resident’s medications, including Allopurinol 100 mg, Aspirin 81 mg, Metoprolol Succinate ER 25 mg, Olmesartan Medoxomil 40 mg, and Pantoprazole Sodium 20 mg. The reportable event form stated the nurse had pre-poured medications for two residents and placed the cups in the top drawer of the medication cart, then failed to verify the correct medication cup before administration. The nurse later gave the wrong cup to the resident after an interruption, resulting in the resident becoming hypotensive and dizzy and requiring transfer to the emergency department. The nurse’s note documented that the resident initially had vital signs within normal limits and was at baseline mentation, but after lunch complained of weakness, appeared pale, and had blood pressure readings that decreased to 81/48 and 110/68 with a pulse of 77. Emergency medical records showed the resident’s blood pressure was 74/40 with a pulse of 57 and sinus bradycardia was noted. Hospital documentation identified hypotension in the ED, improvement with fluids, no significant electrolyte abnormalities, and observation after poison control consultation. Interviews with the nursing supervisor, the charge nurse, and the DON confirmed that the wrong resident’s medications were administered and that the nurse had placed dispensed medications for both residents in the medication cart before giving the incorrect medications.
