Summary

The facility failed to manage pain for a resident with advanced cognitive impairment, anxiety, dementia with agitation, and hospice involvement. The resident required total assistance with care, had a left-hand contracture with splinting/palm guard use, and developed an unstageable coccyx pressure ulcer. The care plan identified the resident as at risk for pain and directed staff to assess, treat, and document pain every shift and as needed, observe for signs and symptoms of pain/discomfort/anxiety, and administer pain medications as ordered. However, the clinical record showed no documented reports of pain on the May 2026 MAR, except for one administration of Morphine Sulfate 5 mg on 5/4/26, and a wound care note on 4/29/26 documented the resident denied pain. During observation of sacral wound care, the resident was noted to be tense, with hands in fists and occasional labored breathing, while the RN did not explain the procedure or converse with the resident during care. Interviews with the wound RN, an NA, and an LPN indicated the resident showed pain or discomfort during wound care, transfers, when being placed in bed, and when the palm guard was applied. Despite these observations and staff reports, the record lacked documentation of pain behaviors and pain medication administration associated with those episodes, and the care plan was only updated after surveyor inquiry to include medicating for pain prior to wound dressing changes.
