Summary

The facility failed to maintain a clinical record that was complete, accurate, and timely for one resident with diagnoses including dementia with anxiety, mild cognitive impairment, schizoaffective disorder-bipolar type, muscle weakness, type II diabetes mellitus, and atrial fibrillation. The resident’s quarterly MDS identified moderately impaired cognition with a BIMS score of 11 and independence with bed mobility and ambulation without an assistive device. The resident care plan identified the resident required staff assistance with ADLs, was a fall risk, and had cognitive impairment, inappropriate behaviors, and resistance to care. On 5/8/26, the resident left the facility and was later found at a hotel after police were contacted. Facility documentation reviewed from 5/8/26 through 6/10/26 did not include documentation that the resident was missing, that missing resident procedures were initiated, that police were notified, or that the resident was returned by the Regional Administrator after police involvement. A nurse’s note instead documented that the resident returned from an LOA, although staff interviews confirmed there was no responsible party approval for an LOA and that the resident had not signed out or been approved to leave independently. The Missing Resident, Unauthorized Leave, and Elopement Response policy required documentation of the missing resident event, search efforts, notifications, assessment upon return, and related follow-up information.
