The ADON stated at 1:43 p.m. that the emergency generator was on, and normal power was restored at 2:51 p.m. During a follow-up interview on 4/24/26, the DM stated the generator company installed the shutdown switch a year ago and that the generator should kick in within 6-9 seconds during a power outage. The DM stated the generator did not kick in within that time because the switch was pushed in, which shut the generator down, and that he had to pull the switch out when he checked it during the outage. He stated someone touched the switch, it was difficult to reset, and it took 10 minutes for the generator to kick in. The DM also stated this had happened in the past and that he had notified the AD and kitchen staff to be careful not to touch the switch. During observation, the generator switch was located to the right of the kitchen back door, and the DM demonstrated how easily the switch button could be pressed. The ADM later stated he had not been told the switch location was a bad spot and had not been notified of an incident related to the switch. The facility policy stated alternate energy sources would be used during a disruption in normal power, and that life-saving equipment and oxygen generating equipment were required to be restored to back-up power within 10 seconds of failure of the normal power source.
