Summary

The facility failed to provide necessary ADL and incontinence care for two dependent residents. One resident had diagnoses including hemiplegia, cerebral infarction, and diabetes mellitus, and was assessed as dependent for toileting, showering, lower body dressing, and shoes. The resident’s care plan directed staff to check and assist with toileting as needed and provide peri care after incontinent episodes, and the record also noted moisture-associated skin damage to the right inner thigh and perianal MASD. During observation, the resident was found with a soiled incontinence brief containing urine and stool while a dressing was present on the sacrococcyx area. A second resident, also diagnosed with hemiplegia and cerebral infarction and assessed as dependent for toileting, showering, lower body dressing, and shoes, had a care plan directing staff to assist with transfers, repositioning, and maintain good personal hygiene every shift and as needed. During observation, the resident was lying in bed with a urine-soiled towel positioned between her legs. Staff stated the resident was incontinent of urine and stool, that incontinence care was to be provided about every two hours and as needed, and that the towel was not part of the resident’s care plan. Staff also stated they could not confirm when the resident had last been checked or changed before the observation. Interviews with staff and family indicated ongoing concerns with delayed incontinence care. A family member reported observing the resident remaining soiled for up to four hours over the prior three months. The UDON and CCO stated dependent residents should be checked and changed every two hours and as needed, and both acknowledged that prolonged exposure to urine and stool could affect skin integrity and that towels were not an approved incontinence management intervention. The facility policy stated it was the facility’s policy to promote skin hygiene, minimize infection risk, and facilitate skin integrity by providing incontinent care as needed.
