Delayed Pregabalin Ordering and Administration

Rio Hondo Subacute & Nursing CenterMontebello, California Survey Completed on 06-30-2026

Summary

The facility failed to ensure the timely availability, ordering, and administration of Pregabalin 50 mg three times daily for one resident with diagnoses including sepsis, muscle weakness, and sciatica. The resident’s admission paperwork and physician orders showed Pregabalin was to continue for neuropathic pain, and the resident was documented as having intact cognition and the capacity to understand and make decisions. The medication was ordered to start on 6/3/2026, but progress notes repeatedly documented that the medication was awaiting delivery, was not available, or was pending the physician’s signature. Record review showed the pharmacy would not release the controlled medication without a signed prescription, yet the facility did not obtain the required physician signature in a timely manner. Notes documented that staff were waiting for pharmacy delivery, that the order was pending the physician’s signature, and that the medication should be held until the physician signed. The resident’s MAR showed multiple missed scheduled doses from 6/3/2026 through 6/15/2026, including entries stating the medication was not administered because it had not been delivered by pharmacy. One MAR entry on 6/9/2026 documented Pregabalin as administered, but later interview indicated it was not actually given. Interviews with family and nursing staff confirmed the delay and the lack of timely follow-up. The family member stated the resident did not receive Pregabalin for more than a week and experienced uncontrolled pain in the lower legs during that time. An LVN stated she notified the primary physician that the medication had not been delivered because the pharmacy required a signed prescription, but she did not contact the medical director and waited several days until the pain specialist came to the facility to sign the order. The LVN also stated there was no documentation that other licensed nurses contacted the physicians on the resident’s behalf during the days the medication remained unavailable. The ADON stated nurses should have contacted the physician immediately and, if unavailable, the medical director or pain specialist, and acknowledged that the resident did not receive the routine pain medication for seven days.

Short Summary

A resident with DM, ESRD, bipolar disorder, and autism had an order for Ritalin 20 mg BID for ADHD, but the medication was not administered for five days and nine scheduled doses were missed. Nursing notes repeatedly documented the drug as pending delivery, pending approval, or pending script, and the MAR showed each dose signed off with Code 9. The physician was not aware the resident had not received the medication, and the DON stated the delay occurred because the facility had to fax the hard copy prescription to the pharmacy.
