Summary

The facility failed to ensure restorative nursing services (RNS) were provided as ordered and as scheduled for three residents reviewed for range of motion and mobility services. Resident 1 was admitted with hemiplegia and hemiparesis following a stroke and had documented ROM limitation. His MD order called for AAROM of both lower extremities daily, five days per week, and the care plan and facility schedule also referenced restorative ambulation with a front-wheeled walker and PROM. However, the POC documentation showed multiple missed sessions for PROM and no documentation at all for the ambulation service. Resident 2 was admitted with a diagnosis of malignant neoplasm of the left breast and had no memory impairment. Her MD order for restorative ambulation with a front-wheeled walker had an active date range, but the facility schedule and care plans did not match the order. The schedule listed AROM with weights and therapy band, as well as additional PROM to the left upper extremity, while the resident’s POC documentation showed several missed restorative sessions and no documentation for ambulation or the PROM service. Resident 3 was admitted with COPD and had ROM limitation in both lower extremities. His MD order called for PROM on all planes three times weekly or as tolerated, but the care plan was inconsistent, the schedule listed a different frequency, and the POC documentation showed multiple missed sessions with no active care plan in place. During interviews, Resident 1 stated he had not been getting RNS while the restorative aide was sick and that missing exercises made him feel further behind. Resident 2 stated she had just started getting exercises again. Resident 3 stated no one had done his exercises during the time the restorative aide was out sick and that the exercises had helped him. The restorative aide confirmed she had been out sick for about a month. The DSD stated the RNS schedule should match the MD order, documentation was required to show care was provided, refusals should be documented, and if care was not documented it was not done. The DSD also confirmed the mismatches between orders, schedules, care plans, and documentation for all three residents.
