Failure to Follow Standardized Texture-Modified Recipes

Gateway Post AcutePorterville, California Survey Completed on 05-14-2026

Summary

The facility failed to ensure a [NAME] followed standardized texture modified recipes for seven residents on pureed or blenderized diets. During a concurrent observation and interview in the kitchen, the cook stated she placed seven pieces of bread in a food processor, then added an unmeasured amount of milk and an unmeasured amount of thickener, and said she was unable to state how much of either ingredient she used because she did not measure them. She stated she was looking for a pudding-like texture when preparing the pureed bread. During another observation, the cook measured six servings of pasta at 4 ounces per serving and placed it into a food processor, then poured an unmeasured amount of milk from a stainless-steel pan into the processor and added thickener from an unlabeled pitcher after visually checking the texture. She transferred the pureed pasta to a separate container for lunch service and stated she was not sure how much milk or thickener she added because she did not measure the ingredients. In a later observation, she added more unmeasured milk to the remaining pureed pasta and blended it further, stating she was processing the pureed bread further for blenderized diet orders and was just looking for the right consistency. The facility's recipe for pureed starches specified measured amounts of warm milk and stabilizer, and the RD stated standardized recipes should have been followed for all texture-modified foods and diet orders.

D
F0804 F804: Ensure food and drink is palatable, attractive, and at a safe and appetizing temperature.
Short Summary

Two residents were served microwave-reheated items without a temperature check before service. A staff member reheated almond milk and chicken noodle soup, placed both on the counter, and they were later measured at 182 F and 190.4 F, respectively. Staff stated the kitchen checked temperatures, but the items were served before confirming they had cooled to a safe temperature.

Short Summary

A resident with cognitive impairment and a mechanically altered diet was left sitting at the dining table with an uncovered lunch tray for nearly 45 minutes without staff cueing or feeding assistance, despite needing supervision and help with meals. When an NA finally began feeding her, the meal items were found to be lukewarm or cold, and neither the NA nor the dietary manager reheated the food or replaced the tray.

F0804 F804: Ensure food and drink is palatable, attractive, and at a safe and appetizing temperature.
Short Summary

Food portions were not served per recipe directions when a cook prepared sandwiches, pureed items, cake, and side items by estimation instead of measured amounts. The cook used tongs to add unmeasured meat, onions, and cheese to buns, served visibly different cake portions, and confirmed there were not enough pickles for all residents. The cook stated the portions were “eye balled” and that the spoons were inconsistent.

E
F0804 F804: Ensure food and drink is palatable, attractive, and at a safe and appetizing temperature.
Short Summary

Food was not prepared to preserve nutritive value for 233 of 237 residents receiving kitchen-prepared meals. E
F0804 F804: Ensure food and drink is palatable, attractive, and at a safe and appetizing temperature.
Short Summary

Food and beverages were not served at proper temperatures and were described as unpalatable during a lunch meal. Tray line issues included no plate warmer in use, cold drinks left unrefrigerated, and delayed tray delivery, and the test tray showed meat loaf, mashed potatoes, vegetables, milk, and coffee at suboptimal temperatures. Residents and an LPN reported the meal was cold, bland, dry, tough, or tasteless, and the FSD and RD gave differing expectations for hot and cold food temperatures.
