Summary

The facility failed to implement and revise comprehensive person-centered care plans for three residents who had low air loss mattress (LALM) orders for skin management or skin and wound management. For Resident 1, the record showed diagnoses including diabetes mellitus with foot ulcer, history of TIA and cerebral infarction, and osteoarthritis of both ankles and feet. The MDS indicated intact cognition, dependence on staff for multiple activities of daily living, maximal assistance for bed mobility, and risk for pressure ulcers/injuries. The order summary showed an LALM with bolsters ordered for skin management, and during observation the resident was lying in bed on the LALM while the treatment nurse stated it was being used for skin management. For Resident 2, the record showed diagnoses including left femur and tibia fractures and osteoarthritis of the left hip. The MDS indicated intact cognition and moderate assistance needed for toileting, personal hygiene, showering, dressing, bed mobility, and transferring. The order summary showed an LALM ordered for skin management, but during interview and record review the treatment nurse stated there were no care plans for the LALM use for skin management and no information about the resident’s LALM setting mode. For Resident 3, the record showed diagnoses including diabetes mellitus, dysphagia, aphasia following cerebral infarction, and anemia. The MDS indicated moderately impaired cognition, maximal assistance for toileting hygiene, showering, lower body dressing, and transferring, moderate assistance for eating, oral hygiene, and bed mobility, and risk for pressure ulcers/injuries. The order summary showed an LALM ordered for skin and wound management with monitoring for functional settings every shift. The existing care plan for LALM, last reviewed on 5/6/2025, included turning and repositioning every two hours and keeping the area clean and dry, but the LVN stated it did not address setting levels or mode, how to manage the LALM with linen, or other specific individualized interventions, and was not person-centered.
