Summary

Facility failed to maintain complete and accurate medical records for three reviewed residents in the behavioral health unit. Resident 1 was admitted with diagnoses including schizoaffective disorder and had a Close Observation Log indicating monitoring for auditory hallucinations and self-harm. During observation and record review, an LVN documented Resident 1 as observed at 1:45 p.m. and 2:00 p.m., but stated during interview that the 1:45 p.m. entry had been made late. The 1:45 p.m. entry did not indicate that it was a late entry. Resident 2 was also admitted with schizoaffective disorder and was being monitored as a new admission to the behavioral health unit. The Close Observation Log showed Resident 2 last observed at 1:30 p.m., but a CNA later charted observations at 1:45 p.m. and 2:00 p.m. and stated those times were entered at 2:13 p.m. The 1:45 p.m. entry did not indicate that it was a late entry. Staff also identified entries on the Close Observation Logs for Residents 1 and 2 that showed observations at future times, and staff stated charting in advance was incorrect documentation. The record review also identified unclear and incorrect documentation related to levels of observation for Residents 1, 2, and 3. An LVN documented a PCP response as recommending 1:1 close observations for Resident 1, but later stated that was incorrect and that the 1:1 monitoring was a nursing decision. The BHD stated several notes for Resident 1 incorrectly reflected 1:1 monitoring when the resident was actually on Q15 monitoring, and that one note was confusing because it did not clearly state the change from 1:1 to Q15. For Resident 2, the BHD stated a Close Observation Log was incomplete because it did not indicate whether the resident was on 1:1 or Q15 monitoring. For Resident 3, the BHD stated charting showed the resident started on 1:1 monitoring, later transitioned to Q15 monitoring, and then another note incorrectly continued to show 1:1 monitoring. The facility policy required errors to be lined through and late entries to be identified as such, and required documentation whenever the level of care changes.
