Summary

The facility failed to provide adequate supervision and implement appropriate interventions to prevent the elopement of a resident who had been identified as at risk for leaving the facility without notice. The resident was admitted with metabolic encephalopathy and had a BIMS score of 10, reflecting moderate cognitive impairment. After a May 28, 2026 incident in which the resident was observed exiting the front door and was redirected back inside, the resident was identified on a LAWN evaluation as being at risk for leaving without notice, with concerns noted for safety awareness and wandering in and out of the facility. Interviews with nursing leadership and staff confirmed that the resident had a history of exit-seeking behavior, that visual checks were being performed but not documented, and that no WanderGuard had been in place before the elopement. The CNA stated she had been checking on the resident approximately every 20 minutes to one hour without documenting those checks, and the DON acknowledged that hourly documented monitoring should have been implemented to verify the resident’s whereabouts.
