Summary

The facility failed to ensure that Resident 6 received tracheostomy suctioning every two hours as ordered. Resident 6 was admitted and re-admitted with diagnoses including acute respiratory failure, aphasia, type 2 DM, transient ischemic attack, and a Stage IV pressure ulcer/injury. The H&P noted fluctuating capacity to understand and make decisions, and the MDS showed the resident was dependent on nursing staff for oral hygiene, toileting, showering, dressing, rolling, transferring, and received respiratory therapy, oxygen therapy, suctioning, and tracheostomy care. The order summary dated 5/15/2026 directed tracheostomy suctioning for airway clearance every two hours as needed. During observation on 6/9/2026, Resident 6 was seen in bed with a tracheostomy tube in place, with loud audible lung sounds, coughing, and flushed red skin coloration. Staff entered the room multiple times while the resident continued coughing, and the wound care nurse was later at the bedside preparing to perform a dressing change. Record review showed the resident was scheduled for tracheostomy suctioning at 10:00 a.m. but did not receive suctioning until 12:22 p.m., more than two hours later. The RT stated he was responsible for suctioning residents every two hours, acknowledged he did not suction the resident at 10:00 a.m., and said nursing supervisors could also perform suctioning. The DON stated it was unsafe for a resident to miss suctioning because the resident could experience respiratory distress.
