Summary

The facility failed to provide sufficient licensed nursing staff to meet resident needs on the night shift. The Facility Assessment identified a required staffing level of one RN and two LVNs for each night shift, but the facility had only one RN assigned and no LVNs on the night shift. The Administrator stated the facility had decided to staff nights with only one RN beginning in September 2025 and had not completed an assessment or evaluation before reducing licensed nursing coverage. The DON and Administrator both acknowledged that the staffing pattern did not match the Facility Assessment. During interviews, CNA 1 stated she was responsible for nine residents during her shift and felt rushed because of staffing shortages, with frequent call-offs occurring on all shifts. She stated residents on the subacute unit were high acuity and dependent on staff for ADLs, incontinence care, and repositioning. LVN 5 stated she was responsible for 13 residents, did not feel able to complete her assignment, and reported that staffing shortages caused delays in medication administration, resident assessments, and charting. She also stated the subacute unit was short-staffed for CNAs daily and that inadequate staffing could delay medications, repositioning, pain management, and increase fall risk. On the night shift reviewed, the RNS was responsible for 47 residents, including approximately 14 residents receiving G-tube feedings and six blood glucose checks. Record review showed enteral feedings, blood glucose monitoring, and oxygen saturation monitoring were completed late. The RNS stated she was the only licensed nurse on duty, had no LVNs assigned, and was unable to complete all required nursing duties, including obtaining vital signs, administering medications on time, and administering enteral feedings on time. The DON stated the facility had not evaluated whether resident care was affected by having only one RN on the night shift and had not reviewed whether medications or treatments were delayed. The Administrator stated the facility had not evaluated the impact of reduced licensed nursing coverage on resident care needs, including medication administration, enteral feeding administration, monitoring, and other required nursing services.
