Summary

The facility failed to ensure that one sampled resident received the physician-ordered regular diet. Resident 2 was admitted with diagnoses including altered mental status, dysphagia, and aphasia. The resident's MDS dated 5/28/2026 indicated severely impaired cognitive skills for daily decision making and dependence for eating, toileting hygiene, showers, upper body dressing, and personal hygiene. The active order summary dated 6/23/2026 showed a NAS diet with regular texture and thin consistency ordered on 4/22/2026. During lunch observation on 6/22/2026, a staff member placed Resident 2's meal tray in front of the resident and Family Member 1, who stated the tray was not correct and that the resident received regular texture. ST 1 removed the tray, returned it to the kitchen, and brought back a tray with the correct regular texture diet. ST 1 later stated the resident had received an incorrect texture meal tray and that giving a diet texture below the prescribed level could cause the resident to refuse the meal or consume less than 100 percent of the meal. The Dietary Services Manager stated Resident 2 received a soft and bite sized diet instead of the prescribed regular texture diet and that staff should verify the diet texture on the tray against the diet order ticket before serving the meal.
