Inaccurate fall risk assessment and missing post-fall care plan updates

Summary

The facility failed to accurately assess and document a resident’s fall risk after a witnessed fall and failed to develop a resident-centered care plan based on that fall. Resident 1 had diagnoses that included repeated falls, hypertensive heart disease with heart failure, and depression, and the MDS dated 5/4/2026 indicated severely impaired cognitive skills for daily decision making. The MDS also showed the resident required supervision or touching assistance for several bed mobility tasks, partial/moderate assistance for transfers and walking, and was dependent for wheeling 50 feet with two turns. After a witnessed fall in the resident’s room while adjusting his body to get more comfortable, the facility’s Fall Risk Evaluation form dated 6/4/2026 documented that the resident had not fallen in the past three months and did not take antihypertensive or psychotropic medications, resulting in a low fall-risk score. The resident’s existing care plan, dated 8/21/2025, identified fall risk related to generalized weakness, impaired cognition, poor safety awareness, poor judgment, poor coordination, medication use, and a history of repeated falls, but it was not revised after the 6/4/2026 fall. There was also no new care plan initiated between 6/4/2026 and 6/9/2026 to address the actual fall. The resident fell again while attempting to self-transfer from his wheelchair to his bed. PT documentation from 6/8/2026 indicated the resident required CGA for sit-to-stand, bed transfer, and wheelchair transfer, and staff interviews stated he needed partial to moderate assistance and supervision while in his wheelchair because he tended to stand or transfer on his own. On 6/9/2026, the resident remained in his wheelchair in the hallway after his family left, wheeled himself to his room, and fell while transferring to bed. Staff interviews confirmed he was not visually monitored at that time, and the DON stated the resident’s fall risk was not accurately assessed and documented and that the post-fall interventions discussed were not added to the resident’s care plan.
