GT Feeding Left Running While HOB Was Lowered

Summary

The facility failed to ensure that a resident receiving GT feeding had the HOB maintained between 30 and 45 degrees while the feeding was running. Resident 4 was admitted with chronic respiratory failure and dysphagia, and the MDS indicated severe cognitive impairment and dependence on staff for ADLs. During observation, a CNA lowered the resident’s HOB to a flat position for pericare, removed the nasal cannula and gown, and began wiping the resident’s face while the GT feeding continued to run. During the same observation, an LVN stated the GT feeding needed to be turned off because the HOB was lowered, and then turned the feeding off. The CNA later stated she did not ask a nurse to turn off the feeding before lowering the HOB and thought it was already off. The DSD stated GT feedings needed to be turned off when the HOB was lowered during pericare. The physician’s order directed the HOB be elevated 30 to 45 degrees at all times while feeding was running, and the facility policy stated the HOB should be raised 30 to 45 degrees during GT feeding and for 1 hour after feeding.
