Missing bed rail orders, consent, assessments, and care plans
Summary
The facility failed to safely use half bed rails for four sampled residents by not completing required documentation and assessments tied to their use. During observation, each of the four residents was seen lying in bed with bilateral 1/2 bed rails elevated. Record review and staff interview showed that the facility did not have the required informed consent, physician's order, bed rail evaluation, or care plan documentation in place for all of the residents as applicable to each case. Resident 14 was admitted and readmitted with ESRD, dependence on renal dialysis, type 2 DM, and diabetic neuropathy. The MDS showed intact cognitive skills for daily decision making and substantial/maximal assistance with several activities of daily living. The resident was observed with bilateral 1/2 bed rails elevated. The OSR showed an order for bed rails as an enabler for turning and positioning, but the Bed Rail Evaluation indicated the resident or representative was not informed about the medical needs addressed by side rails, the benefits and potential side effects, or alternatives that had failed to meet the resident's needs. The MDS Director stated there was no documentation of bed rail informed consent and that the evaluation was incomplete. Resident 77 was admitted and readmitted with CAUTI, stage 4 CKD, and DM. The H&P stated the resident did not have capacity to understand and make decisions, while the MDS indicated intact cognitive skills and substantial/maximal assistance with several ADLs. The resident was observed with bilateral 1/2 bed rails elevated. The OSR had no physician's order for bed rail use, and the care plan had no documentation for bed rail use. Staff stated there was no physician's order, informed consent, or care plan documented for the bed rails. Resident 82 was admitted and readmitted with DM, hypertension, and hypothyroidism. The H&P stated the resident had capacity to understand and make decisions, while the MDS showed moderately impaired cognitive skills and dependence or partial/moderate assistance with several ADLs. The resident was observed with bilateral 1/2 bed rails elevated. The OSR had no physician's order, and the care plan had no documentation for bed rail use. Staff stated there was no physician's order, Bed Rail Evaluation, informed consent, or care plan documented. Resident 108 was admitted and readmitted with DM, depression, and dysphagia. The H&P stated the resident had fluctuating capacity, and the MDS showed moderately impaired cognitive skills with substantial/maximal assistance for several ADLs. The resident was observed with bilateral 1/2 bed rails elevated. The OSR had no physician's order, and the care plan had no documentation for bed rail use. Staff stated there was no physician's order or care plan documented, and the Bed Rail Evaluation was incomplete because the resident or representative was not informed about the medical needs addressed by side rails, the benefits and potential side effects, or alternatives attempted that failed to meet the resident's needs.
