Failure to Monitor LAL Mattress and Complete Bed Safety Assessments

Summary

The facility failed to provide a safe, hazard-free environment for a bedbound resident who required total care and had severe cognitive impairment, hemiplegia/hemiparesis after cerebral infarction, ankylosing spondylitis, atrial fibrillation, and significant dependence for all activities of daily living. The resident had care plans for skin breakdown prevention, feeding tube use, and fall risk, and physician orders for a low air loss (LAL) mattress and for head-of-bed elevation of 30 to 45 degrees during tube feeding. The care plan did not address resident safety while the head of bed was elevated on the LAL mattress and without side rails for bed mobility and safety. The resident’s record showed no documented evidence that staff monitored the LAL mattress settings, placement, or functioning as ordered. The facility also did not complete the required quarterly side rail assessment or the alternatives for bedside rail utilization assessment, and the alternatives assessment was left blank. CNA 1 stated she repeatedly found the resident lying in different positions than the side-lying position she had placed him in, despite believing he could not move independently and required two-person total assistance. CNA 1 stated she did not report these unusual position changes to the charge nurse or supervisors. On the day of the incident, CNA 1 entered the room and observed the resident sliding out of bed headfirst and face down. The resident fell to the floor and sustained lacerations to the forehead and wrist. The resident was later noted to have headache, severe head pain, and loss of vision, and was transferred to the emergency department. CT imaging showed a scalp hematoma, 7 mm leftward midline shift, bilateral nasal bone fractures, and no acute intracranial hemorrhage. Interviews with staff showed disagreement and uncertainty about the LAL mattress model and settings, and the DON stated the facility monitored mattress settings every shift, while the DOM stated the resident’s mattress model did not have a numerical setting display like the one referenced in the chart.
