Summary

The facility failed to provide a variety of fresh fruit options for two of five sampled residents, with only bananas available when residents requested fresh fruit. Resident 2 was admitted with diagnoses including a left femur fracture, right hip dislocation, depression, and anxiety disorder, and had a BIMS score of 15 indicating intact cognition. Resident 2's nutritional evaluation noted regular diet with thin liquids and recommended snacks, supplements, and other support, with comments stating estimated nutritional needs were likely not met due to poor meal intake and inadequate food intake in relation to food preferences. During interview, Resident 2 stated the facility only served bananas and did not have oranges, apples, fresh lettuce, or tomatoes, and that family had to bring food from outside because of the lack of fresh fruit and vegetable options. Resident 5 also stated the food served was poor and confirmed there were no fresh fruits available. The Dietary Manager reviewed the weekly menus and confirmed the facility only had bananas to offer when residents requested fresh fruit options and that there was a lack of variety in fresh fruit options. The weekly menus reviewed showed fresh fruit salad was scheduled only once during the week of 5/31/26, fresh green salad was scheduled twice that week, no fresh fruits or fresh salads were listed on the week of 5/24/26, and no fresh fruits were served at all on the week of 5/17/26. Food supplier invoices showed that during the reviewed weeks, the only fresh fruit item ordered and delivered was bananas.
