Summary

The facility failed to ensure one sampled resident with diagnoses including affective-mood disorder, psychosis, dementia with psychotic disturbance, and schizophrenia-related behavioral symptoms received necessary behavioral health assessments, care, and services. The resident was admitted with a history of mood disorder/psychosis and dementia requiring supportive care, and the baseline care plan identified cognitive impairment with behavioral concerns of agitation and striking out, but it did not include any interventions for those behaviors. The physician later ordered transfer to another hospital for further evaluation and treatment due to uncontrolled restlessness and agitation. The interdisciplinary team documented that psychiatry consults were to be used as needed, and the resident was receiving Quetiapine for psychosis manifested by agitation, Mirtazapine for depression, and Lorazepam as needed for anxiety manifested by restlessness. The facility administrator was unable to state whether the resident should have had a care plan for the behaviors being displayed or whether the facility could provide all necessary care once the resident was accepted. The DON stated the facility became aware of the need for a 1-to-1 sitter less than 24 hours after admission and expressed concerns about safety for staff and the resident. The resident was then sent back to the hospital because the facility was unable to accommodate the 1-to-1 sitter requirement. The report also cited the facility's baseline care plan policy requiring immediate health and safety needs to be addressed and a person-centered care plan developed no later than 21 days after admission.
