Summary

The facility failed to prevent the development of an unstageable pressure injury on a resident’s left foot/heel after admission with a left ankle-foot orthosis (AFO) boot following surgery for a left ankle-foot fracture. The resident also had type 2 DM and diabetic polyneuropathy. The hospital summary documented the resident as non-weight bearing on the left foot, with the surgical site intact and the left lower foot wrapped with an ACE bandage, but the record did not reflect specific instructions for surgical care of the left foot. The physician orders and nursing admission assessment also did not include treatment specific to the left foot, and the admission assessment documented a soft cast/AFO with no description of the surgical incision site or related treatments. The skin integrity care plan identified risk for pressure ulcer and skin impairment related to decreased mobility, DM2, polyneuropathy, insulin use, and opioid use, and included weekly treatment documentation for any skin breakdown. However, weekly assessments from admission through 6/11/26 did not indicate skin issues. During interview, the resident stated staff did not remove the AFO to assess the skin from admission until 6/11/26 and that the boot remained on for weeks despite repeated complaints of left foot and ankle pain. The resident also became tearful and stated fear of gangrene or amputation because of diabetes and lack of care. A change of condition on 6/11/26 documented a pressure ulcer/injury to the left heel acquired in-house. A wound consult on 6/16/26 identified a left heel medical device-related pressure injury at an unstageable stage, measuring 2.5 cm by 3 cm with 15% eschar. The RN confirmed that the resident was admitted with a surgical wound, ACE wrap, and AFO, but no physician orders for left heel treatment or wound consult were obtained during admission, and skin assessments were done by checking toes and looking for redness or warmth around the AFO without removing the ACE bandage or AFO. The RN and RNS confirmed the AFO was left on day and night, was not removed during daily or weekly skin assessments, and no physician orders for AFO or surgical incision care were obtained until after the wound was discovered.
