Oxygen Therapy Not Provided Per Orders
Summary
The facility failed to provide respiratory care consistent with physician orders and facility policy for two residents who were receiving oxygen therapy. Resident 9 had diagnoses that included COPD, chronic respiratory failure with hypoxia, CMML, and PE. The resident’s records showed a physician order for continuous oxygen at 4 LPM via nasal cannula, and the care plan identified continuous oxygen therapy at 4 LPM. However, during observations on 6/1/2026 and 6/2/2026, Resident 9 was found receiving oxygen at 2 LPM via nasal cannula instead of the ordered 4 LPM. During the 6/2/2026 observation, CNA staff stated the resident was usually on oxygen at 4 LPM, and the oxygen concentrator was observed set at 2 LPM. RN 3 reviewed the order and stated the resident should have been receiving 4 LPM, then entered the room and confirmed the oxygen was set at 2 LPM. RN 3 stated she had been in the room earlier and had not checked the concentrator setting. The DON later reviewed the oxygen administration policy and stated supplemental oxygen requires a physician’s order that includes the flow rate in LPM, and that the policy was not followed when Resident 9’s oxygen was not administered per the physician’s orders. Resident 114 had diagnoses that included acute respiratory failure with hypoxia, pulmonary edema, PE, and interstitial pulmonary disease. The record showed a completed order for initial oxygen at 2 LPM via nasal cannula for one day, but there was no active physician order to continue oxygen from 5/26/2026 through 6/1/2026. Despite this, the resident was observed receiving oxygen at 2 LPM via nasal cannula, and the chart contained oxygen saturation documentation on multiple dates while oxygen was in use. LVN 3 stated oxygen is considered a medication and requires a physician’s order, and that Resident 114 should have had an order to administer oxygen during that period. MDSC 1 stated there was no documented evidence that Resident 114 was monitored while on oxygen during that time, and the DON stated the facility policy requiring a physician’s order and monitoring was not followed.
