Failure to Resubmit PASARR Screening After Case Closure

Santa Clarita Post-acute Care CenterNewhall, California Survey Completed on 06-05-2026

Summary

The facility failed to ensure that a resident with a positive PASARR Level I screening received the required Level II PASARR evaluation and determination. Resident 11 was admitted with diagnoses including unspecified psychosis, unspecified dementia, and falling. The resident's H&P stated the resident did not have the capacity to understand and make decisions, and the MDS showed the resident rarely or never could make herself understood, sometimes understood others, was dependent on staff for multiple ADLs, and was taking antipsychotic medication. Resident 11's PASARR Level I screening was positive for serious mental illness, and the notice stated that a Level II mental health evaluation was required. The notice further stated the facility would be contacted within two to four days to arrange an evaluator to review the resident's records and possibly complete a telephone contact or visit. A subsequent notice stated the facility staff were unresponsive to two or more separate attempts of communication within 48 hours of the Level I screening, so the case was closed and a new Level I screening had to be resubmitted to reopen the case. During interview, the MDS Coordinator stated that when the case was closed, one of the licensed nurses with access to the PASARR system should have resubmitted the Level I screening, but it was not done. The MDS Coordinator stated she was one of the nurses who could have resubmitted it, but she was not aware of the notice of attempted evaluation. The DON reviewed the facility PASARR policy and stated the policy was not followed when the facility did not follow up on the notice of attempted evaluation, which potentially resulted in a delay of specific interventions for mental, emotional, and psychosocial care for Resident 11.

D
F0645 F645: PASARR screening for Mental disorders or Intellectual Disabilities
Short Summary

PASARR screening was inaccurate for two residents. One resident had bipolar disorder and a Level I PASARR showing a need for Level II review, but the required Level II evaluation was not obtained before or after admission. Another resident was admitted with major depressive disorder, personality disorder, substance abuse, and PTSD, and the Level I PASARR also indicated a Level II evaluation was required prior to admission; the DON later confirmed the PASARR was incorrect and should have been caught on admission and during audit review.
