Infection Control Practices Not Followed for Medication Equipment and Resident Care

Angels Nursing Health CenterLos Angeles, California Survey Completed on 06-11-2026

Summary

The facility failed to implement infection control practices for Medication Cart 2 and for two residents. During observation on 6/9/2026, a pill cutter in Medication Cart 2 contained white particles and a piece of an unknown tablet left inside it. LVN 3 stated licensed nurses were supposed to clean the pill cutter after each use and discard the remaining tablet per facility policy. The DON later stated the pill cutter should be cleaned between uses and the remaining tablet should be disposed of per policy because of possible cross contamination and because it was not safe for residents. Resident 34 was admitted with diagnoses including COPD, pneumonia, and CHF. The resident’s MAR showed an order for Ipratropium Bromide inhalation treatment as needed for wheezing or shortness of breath, and the MDS indicated intact cognition and need for moderate to supervision-level assistance with ADLs. During observation on 6/8/2026, the resident’s breathing treatment mask and tubing were found on the floor and were unlabeled with a date. LVN 1 stated the mask and tubing were supposed to be inside a bag and labeled with the date when opened, and that the setup was supposed to be changed weekly due to infection control. The DON stated the mask and tubing were not supposed to be on the floor and were supposed to be inside a respiratory bag and dated. Resident 59 was admitted with ESRD and dependence on hemodialysis. The order summary indicated the resident was on enhanced barrier precautions and that staff must wear gowns and gloves during high-contact care activities. The H&P stated the resident had capacity to make decisions, and the MDS indicated intact cognition and mostly moderate assistance with ADLs. During observation on 6/10/2026, LVN 2 checked the resident’s blood pressure without wearing a gown and gloves, and LVN 2 stated the resident needed EBP and that gown and gloves were supposed to be worn when checking blood pressure. RN 1 also stated that gown and gloves were supposed to be worn due to possible risk of infection.

Short Summary

Failure to Use EBP PPE During Wound Care: A resident with a wound and severely impaired cognition was receiving ordered wound care when the DON and ADON entered the room, washed hands, and donned gloves but did not wear gowns before starting care. EBP signage and PPE were present at the bedside, and both leaders later stated they forgot to put on gowns even though the resident was on EBP for an open wound and the facility policy required gown and glove use for wound care.

Failure to Follow Enhanced Barrier Precautions and Hand Hygiene During Resident Care
D
F0880 F880: Provide and implement an infection prevention and control program.
Short Summary

Staff failed to consistently follow EBP and hand hygiene for two residents who required high-contact care. One resident with an indwelling catheter and cognitive impairment did not have EBP followed during transfers, clothing removal, and pericare, and another resident with a suprapubic catheter and multiple pressure ulcers had staff wear PPE incorrectly, remove PPE in the room without hand hygiene, and continue care after emptying the catheter bag without changing gloves or cleaning hands. Interviews confirmed staff were expected to use gowns, gloves, and hand hygiene for these tasks.
