Kingsburg CenterKingsburg, California Survey Completed on 06-05-2026

Summary

The facility failed to store, prepare, distribute, and serve food in accordance with professional standards for food service safety when a dietary aide was observed working in the kitchen with an exposed mustache and facial hair that was not restrained. The Registered Dietitian Consultant later stated that staff were expected to wear hair restraints and facial hair coverings anywhere and anytime they were in the kitchen, and the facility policy required all staff members to have hair confined in a hair net or cap with facial hair properly restrained. The facility also failed to keep foodservice equipment clean and sanitary when a [Brand name] lid was observed on a standing rack with leftover food residue on it. The Dietary Account Manager stated the lid was supposed to be dry, clean, and ready for use, and acknowledged that leftover food residue remained on the equipment. The facility policy required all foodservice equipment to be clean, sanitary, and in proper working order, and the food preparation policy stated staff were responsible for avoiding contamination by physical, biological, and chemical sources.
