Summary

The facility failed to ensure a resident at risk for falls was supervised while using the restroom, resulting in an unwitnessed fall. The resident had diagnoses including fall with injury, a history of falls, and no capacity to understand and make decisions. On the day of the incident, a CNA assisted the resident to the toilet, offered a wheelchair, but the resident insisted on using a walker and requested privacy. The CNA instructed the resident to use the call light when finished and left the area; the resident later stood up, washed her hands, felt dizzy, fell backward, and hit her head. Staff interviews showed the CNA did not check on the resident while she was in the bathroom, and multiple staff members stated they had been trained to stay with residents in the restroom or remain nearby. The facility also failed to complete required post-fall neurological assessments for one resident after an unwitnessed fall. The neurological assessment flow sheet initiated after the fall did not show completion of the 30-minute assessment, all four hourly assessments, or the eight-hour assessment. In addition, the resident's record did not contain documented evidence that a fall risk evaluation was completed on admission. The DON verified that the fall risk evaluation was never completed on admission. A third resident's record also showed incomplete post-fall neurological monitoring after an unwitnessed fall. The neurological assessment flow sheet did not show completion of the one-hour assessment or the four-hour assessments, and the form did not reflect completion for the full 72-hour period, with the last entry documented before the monitoring period was complete. An RN verified the expected neurological monitoring schedule, and the DON verified that the neurological assessments for the two residents were not accurately and completely documented.
