Failure to Use EBP PPE During Wound Care

Summary

Treatment Nurse 1 failed to follow the facility’s Enhanced Barrier Precautions and Infection Control policies while providing wound care to a resident with multiple wounds. The resident was admitted with sepsis, Type 2 DM, and PVD, and had orders for daily wound care to a sacrum pressure injury, a left gluteus pressure injury, a left gluteal fold pressure injury, and a left above-the-knee amputation wound. The care plans identified these wounds as stage 3 pressure injuries and included daily wound care and monitoring for signs and symptoms of infection such as redness, drainage, pain, and warmth. During observation, an EBP sign was posted outside the resident’s room indicating that residents with a blue dot next to their name plate required EBP during high-contact activities. TXN 1 was observed preparing the resident’s wound care supplies and then entering the room without wearing a gown. For about 45 minutes, TXN 1 performed wound care to the left gluteal fold, left gluteus, sacrum, and left above-the-knee amputation wound without wearing a gown.
