Failure to Develop Baseline Care Plan Within Required Timeframe
Summary
The facility failed to develop a baseline care plan for a resident after readmission, and the plan was not initiated within 48 hours as required by the facility policy and procedures titled Person-Centered Care Planning. The resident was readmitted on [DATE], and the record review showed no baseline care plan in the electronic health record. During interviews, the LVN and DON both stated they could not find the baseline care plan in PCC and confirmed it should have been completed within 48 hours of readmission. Resident 1 had multiple significant diagnoses, including muscle weakness, cognitive communication deficit, epilepsy, atherosclerotic heart disease, unspecified dementia, contracture, quadriplegia, anoxic brain damage, TIA, schizophrenia, and major depressive disorder. The H&P dated 4/15/2026 stated the resident was at significant risk of worsening medical status and readmission to the hospital and had waxing and waning capacity for medical decision making due to dementia. The MDS dated 4/20/2026 indicated the resident was cognitively impaired, used a manual wheelchair, had functional limitations in range of motion in both upper and lower extremities, was completely dependent on staff for ADLs, and was incontinent of bowel and bladder. During the concurrent interview and record review, the LVN stated the baseline care plan would identify the resident's needs and the interventions needed, and that without it staff would not know the resident's needs and might miss something important. The DON stated the baseline care plan tells staff what help the resident will need and what interventions are needed, and that without it immediate interventions would be missed and care would be delayed. The facility policy stated the baseline care plan must include the minimum healthcare information necessary to properly care for each resident immediately upon admission and address resident-specific health and safety concerns, and that it must be developed within 48 hours of admission.
