Missing Nursing Documentation for Resident Behaviors, Hospital Transfer, and Nurse Assignment Request
Summary
The facility failed to document nursing assessments, observations, and progress notes for two residents. For one resident, the record showed multiple diagnoses including dementia, schizophrenia, epilepsy, quadriplegia, anoxic brain damage, and total dependence on staff for ADLs, with urinary and bowel incontinence. The care plan identified behavioral problems including throwing feces on the floor and tearing apart the incontinent brief, with interventions to monitor behavior, determine underlying causes, and provide frequent care and hygiene. During interview, the resident could not answer simple orientation questions, and nursing staff and the DON stated they could not find any nursing observation or progress note documentation in PCC for the resident’s feces-throwing or brief-tearing behaviors during May and June 2026. For the second resident, the record showed diagnoses including multiple sclerosis, paraplegia, polyneuropathy, chronic pain syndrome, depression, anxiety, muscular dystrophy, cognitive communication deficit, and dependence on staff for emotional, intellectual, physical, and social needs. The nursing progress notes did not contain licensed staff documentation explaining why the resident was transferred to a general acute care hospital, including the reason for transfer, the date and time of transfer, vital signs before transfer, or the date and time the physician and family/responsible party were notified. Staff interviewed stated they could not find documentation in PCC regarding the hospital transfer. The second resident also reported telling the SSD that a specific LVN was unprofessional, disrespectful, and should no longer be assigned to the resident’s care. The resident stated this request was made more than once, but nursing progress notes from May and June 2026 did not contain documentation of the request. The DON stated the request was known to staff and that the resident had the right to choose another nurse, but there was no documentation in PCC showing the request or the follow-up. The assignment sheets showed the same LVN continued to be assigned on some shifts after the request was made.
