Incorrect application of a resident’s right-hand splint by RNA staff

Summary

The facility failed to ensure a Restorative Nursing Assistant was competent to provide splinting services for one resident when the assistant applied the resident’s right-hand splint incorrectly and did not use the custom splint that had been ordered. The resident had been admitted to the facility with diagnoses including myelin oligodendrocyte glycoprotein antibody disease, chronic respiratory failure, and muscle spasms, and the Minimum Data Set indicated severely impaired cognitive skills, functional limitations in range of motion of both arms and legs, and dependence on staff for hygiene, bathing, dressing, rolling, and bed-to-chair transfers. The resident had a physician’s order for RNA staff to don and doff resting hand splints to both hands for four to six hours, seven times a week. During observation of the RNA session, the resident was lying in bed with the right arm straight and hyperextended at the elbow, the wrist bent downward, and the hand in a fist. The RNA assisted with passive range of motion to the resident’s right arm but was unable to bend the elbow, straighten the wrist, or straighten the thumb, which remained bent inward across the palm. The RNA then placed a resting hand splint on the right hand with the thumb tucked across the palm and secured the splint over the knuckles and in the webspace between the thumb and pointer finger. The RNA stated the resident had previously used a different right-hand splint that was lost about a month earlier and said the current splint was found at the bedside. Occupational Therapy later observed the splint and stated it did not fit properly because the thumb was positioned underneath the hand and across the palm instead of away from the palm. OT stated the resident had previously been evaluated and issued a custom right-hand splint to support the thumb away from the palm, but that splint was lost. The Director of Rehabilitation and the DON stated that splint selection, wear tolerance, and wear schedule required assessment by licensed therapy staff, and that RNAs were not qualified to modify the splinting plan or apply a different splint without a formal therapy assessment.
