Failure to Provide Meatal Care for Residents With Urinary Catheters
Summary
The facility failed to provide meatal care for four residents with urinary catheters. The deficiency involved Resident 3, Resident 4, Resident 6, and Resident 12, all of whom had indwelling urinary catheters and orders related to catheter management. The report states that meatal care was not being performed or documented as expected, and that staff interviewed during the investigation were unclear about who was responsible for cleaning and assessing the catheter meatus. Resident 3 had diagnoses including prostate cancer and benign prostatic hyperplasia with lower urinary symptoms, and his MDS showed moderate cognitive impairment. His initial catheter order on 6/3/26 directed Foley catheter changes as needed, but no meatal care order was present until 6/9/26, when an order was entered to cleanse the meatus with soap and water every shift and monitor for signs and symptoms of infection. During interview, Resident 3 stated he could not remember the last time someone cleaned his meatus or provided catheter care, saying it had not been for a while. Resident 4 had diagnoses including urinary tract infections and COPD, with severe cognitive impairment and dependence for toileting hygiene. His catheter order from 4/21/26 did not include meatal care, and the order for Foley/SP catheter care with cleansing every shift was not entered until 6/9/26. Resident 4 stated he did not remember the last time someone cleaned his meatus or did catheter care. Resident 6 had a diagnosis of urinary tract infections, moderate cognitive impairment, and dependence for toileting hygiene. His catheter order from 4/30/26 directed the Foley catheter to be changed every 2 weeks as needed for clogging or dislodgement, but no catheter care order was present until 6/9/26, when an order was entered for Foley catheter care every shift. Resident 6 stated the nurses do not clean his urethral meatus every shift and that sometimes they come to clean it, but not every shift. Resident 12 had diagnoses including acute respiratory failure and edema. His catheter order from 6/26/25 directed the urinary catheter bag to be changed when the Foley was changed and as needed, but no other catheter care orders were present until 6/15/26, when an order was entered for S/P catheter care with cleansing of the insertion site with mild soap and water every shift. Interviews with nursing staff and leadership showed inconsistent understanding of catheter care responsibilities, and the DON stated that nurses should monitor, assess, and clean the resident's meatus with soap and water every shift, and that if catheter care was not charted, it was not done.
