Infection Control Failures With EBP, Glucometer Disinfection, and Oxygen Equipment Storage

Springs Road HealthcareVallejo, California Survey Completed on 06-12-2026

Summary

The facility failed to ensure proper infection control practices were followed when a Licensed Nurse entered a resident’s room under Enhanced Barrier Precautions without wearing a gown and gloves. The resident had diagnoses including a history of infectious and parasitic diseases and type 2 diabetes, and the care plan and orders reflected a left calf pressure injury and left knee incision sites being monitored for signs and symptoms of infection. During observation, the nurse entered the room, touched the resident’s pillow and left shoulder, and assisted the resident to reposition in bed without the required gown and gloves, then did not sanitize her hands after providing direct care. The nurse acknowledged the resident was on EBP precautions and stated she should have practiced infection control to prevent spread of infection to other residents. The facility also failed to follow the manufacturer’s instructions for disinfecting a glucometer used to check a resident’s blood sugar. The resident had been admitted with bacteremia and type 2 diabetes, and had an order for blood sugar checks before meals and at bedtime. During observation, a nurse used the facility glucometer to check the resident’s blood sugar and then cleaned the device with wipes from the medication cart. The nurse stated she used those wipes all the time after using the glucometer and did not know what other nursing staff used. The DON stated the facility did not use the manufacturer’s recommendation to disinfect the glucometer and used other sanitizing wipes to clean it. The facility also failed to store a resident’s nasal cannula in a bag when not in use. The resident had COPD with exacerbation and acute respiratory failure with hypoxia and had an order for oxygen via nasal cannula as needed to keep oxygen saturation above 90%. During observation, the nasal cannula was resting on top of the oxygen concentrator and not in a bag. The nurse confirmed it was not stored in a bag and stated it should be kept in a bag when not in use. The IP stated the nasal cannula should be stored in a bag when not in use for infection control purposes, and the facility policy indicated oxygen cannulae and tubing used PRN should be kept in a plastic bag when not in use.

Infection Control Practices Not Consistently Implemented for Resident Evaluated for C. difficile
D
F0880 F880: Provide and implement an infection prevention and control program.
Short Summary

A resident being evaluated for C. difficile was not consistently managed under the correct contact-enteric precautions. Staff gave care with incomplete understanding of the precautions, the room signage did not clearly identify the needed disinfectant or contact time, bleach wipes were not always available, and staff were observed missing hand hygiene and handling items under PPE during care.

Failure to Use EBP PPE During Wound Care
D
F0880 F880: Provide and implement an infection prevention and control program.
Short Summary

Failure to Use EBP PPE During Wound Care: A resident with a wound and severely impaired cognition was receiving ordered wound care when the DON and ADON entered the room, washed hands, and donned gloves but did not wear gowns before starting care. EBP signage and PPE were present at the bedside, and both leaders later stated they forgot to put on gowns even though the resident was on EBP for an open wound and the facility policy required gown and glove use for wound care.

Failure to Follow Enhanced Barrier Precautions and Hand Hygiene During Resident Care
D
F0880 F880: Provide and implement an infection prevention and control program.
Short Summary

Staff failed to consistently follow EBP and hand hygiene for two residents who required high-contact care. One resident with an indwelling catheter and cognitive impairment did not have EBP followed during transfers, clothing removal, and pericare, and another resident with a suprapubic catheter and multiple pressure ulcers had staff wear PPE incorrectly, remove PPE in the room without hand hygiene, and continue care after emptying the catheter bag without changing gloves or cleaning hands. Interviews confirmed staff were expected to use gowns, gloves, and hand hygiene for these tasks.

Failure to Use Required PPE for Resident on Contact Precautions
D
F0880 F880: Provide and implement an infection prevention and control program.
Short Summary

A resident on Contact Precautions for active C. difficile infection was not consistently protected by required PPE. Although signage outside the room directed all entrants to wear a gown and gloves, an RN entered the room with medications without either item and touched the bedside table and door surface. The resident stated staff did not always wear gowns, and the DON confirmed staff were expected to wear a gown and gloves when entering the room while precautions were in place.

Infection Control Lapses During Incontinence and Ostomy Care
E
F0880 F880: Provide and implement an infection prevention and control program.
Short Summary

A facility failed to maintain infection control during care for two residents. During incontinence care, a CNA removed gloves and applied clean gloves without hand hygiene before placing a clean brief on a resident who was dependent for toileting hygiene and always incontinent. During ostomy care, an LVN handled a resident’s colostomy, wiped stool from the stoma, and continued care without removing soiled gloves, sanitizing hands, or putting on clean gloves. The DON stated staff were expected to perform hand hygiene between glove changes.

Infection Control Failures During Incontinent Care
D
F0880 F880: Provide and implement an infection prevention and control program.
Short Summary

Infection Control Failures During Incontinent Care: Two residents were observed receiving incontinent care with multiple infection control lapses. A CNA did not perform hand hygiene between glove changes or after glove removal while caring for one resident, and another CNA did not wash hands before care, changed nothing between dirty and clean tasks, touched clean items with dirty gloves, placed soiled linens on the floor, and handled dirty linens with bare hands after glove removal. The DON stated hand hygiene, glove changes, and proper handling of soiled linens were required, and facility policies reflected those practices.
