Missed Monitoring and Documentation for Multiple Residents
Summary
The facility failed to ensure that three sampled residents were adequately monitored for changes in condition and adverse effects of medications. Resident 1 had diagnoses including colon cancer, muscle weakness, seizure, and hypotension, and the MDS indicated the resident’s cognition was not intact and the resident was unable to make decisions. Physician orders included pain monitoring, seizure activity monitoring, monitoring for signs and symptoms, hours of sleep monitoring, and anti-convulsant and anti-depressant monitoring, but the MAR showed multiple missed entries for pain monitoring, seizure monitoring, vital signs, anti-depressant monitoring, hours of sleep, and anti-convulsant monitoring on several dates and shifts. Resident 2 had diagnoses including muscle weakness, seizure, hypertension, diabetes, and history of fall, and the MDS indicated cognition was intact. Physician orders included monitoring for anti-anxiety, anti-convulsant, and anti-depressant medication side effects, monitoring for opioid overdose symptoms, observing for seizure episodes, pain monitoring, and abnormal signs and symptoms every shift including blood pressure, temperature, pulse, and respiratory rate. The MAR showed missed documentation for anti-anxiety, anti-convulsant, anti-depressant, opioid overdose symptom monitoring, seizure observation, pain monitoring, and vital signs on multiple dates and shifts. Resident 3 had diagnoses including cellulitis of both lower limbs, diabetes, and hypertension, and the MDS indicated cognition was intact. Physician orders included insulin administration with finger stick blood glucose checks before meals and at bedtime, pulse oximetry every shift, pain monitoring every shift, monitoring for opioid overdose symptoms, anticoagulant monitoring, and abnormal signs and symptoms every shift including blood pressure, temperature, pulse, and respiratory rate. The MAR showed missed finger stick blood glucose checks, pulse oxygen monitoring, pain monitoring, opioid overdose symptom monitoring, anticoagulant monitoring, and vital signs on multiple dates and shifts. The DON acknowledged that multiple assessments for the sampled residents were not completed and that no documentation or progress notes were done for the missing dates and times.
