Incomplete Care Plans and Missed Ordered Medications and Treatments
Summary
The facility failed to develop and implement person-centered care plans for three residents. For one resident with colon cancer, muscle weakness, seizures, and hypotension, the care plan entries were not specific or individualized. The care plan documented interventions such as administering medication as ordered, checking range of motion, continuing interventions on the at-risk plan, taking vital signs, and monitoring for weight loss, but the interventions did not specify the medication, frequency, or exact parameters to follow. The resident’s MDS indicated impaired cognition and inability to make decisions. The facility also failed to implement ordered medication interventions for two residents with seizure disorders and other diagnoses. For one resident, physician orders included Keppra, Lacosamide, and Depakote, and the care plan directed administration of these medications as ordered, but the MAR showed multiple missed doses of Depakote, Lacosamide, and Keppra in May 2026. For another resident with muscle weakness, seizure disorder, hypertension, diabetes, and a history of falls, physician orders included Lamotrigine, Levetiracetam ER, and Pregabalin, and the care plan directed medication administration in accordance with physician orders, but the MAR showed missed doses of Lamotrigine, Levetiracetam ER, and Pregabalin across May and June 2026. The facility further failed to implement ordered diabetes monitoring and treatment for a resident with diabetes and cellulitis. The physician ordered insulin aspart by sliding scale before meals and at bedtime, and the care plan directed blood glucose monitoring and administration of antidiabetic medication as ordered. The MAR showed that blood glucose was not monitored and insulin was not administered on multiple occasions. In addition, for the resident with bilateral lower extremity cellulitis and skin breakdown, physician orders required Polysporin to both lower extremities and wound care to the left lower leg venous ulcer and right lower leg blister, but the TAR showed multiple missed treatment administrations. Interviews with LVN 1 and the ADON confirmed that the care plans and treatment orders were not followed and implemented as ordered.
