Stage 3 Sacral Pressure Ulcer Linked to Missed Repositioning and Skin Checks

Summary

Resident 4 developed a stage 3 pressure ulcer on the sacrum/coccyx area while at the facility. The resident had multiple risk factors documented in the record, including hemiplegia and hemiparesis following a cerebral infarction, muscle weakness and wasting, severe protein-calorie malnutrition, a left below-knee amputation, peripheral vascular disease, dysphagia, an indwelling catheter, and bowel incontinence. The admission skin assessment noted an ulcer on the left hip, and the Braden Scale on 4/7/2026 and 4/14/2026 identified the resident as at risk for pressure ulcers. The facility’s care plan for impaired skin integrity related to immobility, incontinence, poor perfusion, and malnutrition directed staff to reposition the resident every 2 hours and as needed, assess skin every shift for breakdown, keep skin clean and dry after each incontinence episode, and offload pressure to bony areas. However, the Nursing Weekly Summary for 4/8/2026, 4/15/2026, 4/22/2026, and 4/29/2026 did not indicate the resident was included on the turning and repositioning program or that skin was assessed. The CNA Shower Review form from 4/7/2026 through 5/5/2026 had blank visual assessment sections, and the ADL Repositioning Rounds Monitoring log for the afternoon and night shifts on 5/3/2026 and 5/3/2026 to 5/4/2026 did not include the resident or show that the resident was turned or repositioned every 2 hours. The every-shift documentation report for 5/3/2026 and 5/5/2026 day shifts had blank sections for turning/repositioning and skin observation. On 5/4/2026, the SBAR documented a stage 3 pressure injury at the sacrum/coccyx area and stated the condition was worsened by not being repositioned every 2 hours or changed when soiled. The PA wound progress note also documented a newly developed stage 3 sacral pressure ulcer acquired on 5/4/2026, measuring 4 cm by 4 cm with 0.10 cm depth and areas of granulation and slough. During interview, the LVN stated the wound developed from lack of turning and prolonged pressure and that CNAs did not check the resident’s skin and report changes during ADL care.
