Summary

The facility failed to ensure that Resident 5’s care plan was updated within 7 days of the comprehensive assessment and revised by the interdisciplinary team to reflect the physician’s order for 1:1 supervision after a resident-to-resident altercation. Resident 5 was admitted with diagnoses including left-sided hemiplegia following cerebral infarction, schizophrenia, and psychosis not due to a substance or known physiological condition. The MDS dated 3/27/2026 indicated Resident 5 had no cognitive impairment and required supervision/touch assistance to wheel her wheelchair. A Change of Condition assessment dated 6/16/2026 documented that Resident 5 was aggressive toward others and was involved in an altercation in which she pulled another resident to the floor. A physician order dated 6/16/2026 directed 1:1 supervision from 6/16/2026 to 6/22/2026 because of the physical altercation. During interview, the DON stated care plans were revised after resident-to-resident altercations and that the interventions should be included on the care plan so staff would be aware of them and implement them. The DON confirmed that Resident 5’s 1:1 supervision intervention was not included in her care plan.
