Kitchen Equipment Not Kept Clean and Sanitary

Evergreen Living Center At StagecoachBryant, Arkansas Survey Completed on 06-04-2026

Summary

The facility failed to ensure that food preparation equipment and the kitchen environment were maintained in a clean and sanitary condition. During a concurrent observation and interview with the Dietary Manager, the drip pan was observed covered in thick crusty patches with amber to dark brown substance and dried peas and zucchini, the conveyor toaster had fine dry particulates inside with dark residue on the conveyor mechanism and visible smudges and grease on the exterior, and the oven was heavily soiled with black and gray substances and a yellowish-brown residue where the door closed, with a black and gray substance on the wire rack. The Dietary Manager stated the drip pan was cleaned weekly, the toaster was wiped down after each use but did not come apart for inside cleaning, and the oven was cleaned once a week on Sundays. Staff interviews did not include cleaning of the drip pans or oven, and the weekly task sheets did not show tasks for cleaning the drip trays or conveyor toaster system. The facility’s in-services for the last year did not show training on cleaning and sanitization, while the manufacturer’s manual and facility policy both described cleaning of the toaster components, drip tray, and oven debris or spills as part of routine cleaning.

A Cook said she removed the chicken from the freezer about an hour earlier, and the DM confirmed the chicken was thawed in the sink after staff forgot to remove it from refrigeration earlier. The facility's policy required thawing in refrigeration, under running water, in a microwave under specific conditions, or as part of the conventional cooking process.
