Unsafe Mechanical Lift Transfer Resulted in Resident Injury

Summary

The facility failed to ensure a resident was free from neglect when staff used a manually operated mechanical lift device for a transfer that resulted in a right leg laceration requiring emergency treatment. The resident had been admitted with diagnoses including metabolic encephalopathy, pyelonephritis, urinary device fitting/adjustment, and hypertensive heart disease with heart failure. Her MDS assessment showed a BIMS score of 9, indicating moderately impaired cognition, and her care plan identified her as at increased risk for verbal abuse, physical abuse, sexual abuse, and/or exploitation related to physical, cognitive, or psychosocial deficits. During the evening transfer from wheelchair to bed, two CNAs used the manually operated lift device. Nursing documentation stated the device fell sideways onto the resident’s leg, causing a large laceration with active bleeding. The resident was sent to the ER by 911 and later returned with sutures. The resident stated that the staff used an old hand pump lift, that it tilted during the transfer, and that one CNA went to the other side of the bed and pulled or touched the lift because the legs would not go under the bed; she stated that this was when the handle of the lift hit her leg. The resident’s representative stated that staff called 911 because the wound was gushing blood. Staff interviews and observations showed that the CNAs involved stated they knew how to use the manually operated lift and that two staff were needed for safe use. The facility’s maintenance supervisor and DON later demonstrated the same lift with a similar bed frame, and the lift’s base legs could not go under the bed and the hook could not go over the surface of the bed. The DON stated she would not use the lift because it would be unsafe for transfer under those conditions. The facility’s own manual stated that if the patient cannot be lifted above the bed, staff should not push or pull the patient onto the bed because it can cause a tip over and serious injury. The DON also stated the CNAs may not have had the lift legs spread enough and that they were trying to put the resident in bed quickly.
