Summary

The facility failed to ensure expired medications were discarded for one resident. Resident #109 was admitted and readmitted with diagnoses including seizures, atrial fibrillation, hemiplegia and hemiparesis following cerebral infarction affecting the right dominant side, acute respiratory failure with hypoxia, and hypertensive heart disease without heart failure. The order summary showed an order dated September 9, 2025 for Ativan Solution 2 mg/mL (Lorazepam), 1 mL intramuscularly every 6 hours as needed for seizure activity, with the order later listed as discontinued and no end date indicated. During an observation of the Central Section medication storage room, a vial of Lorazepam with an expiration date of December 2025 was found stored in the medication storage refrigerator inside the controlled medications lock box. Staff interviews confirmed that the medication was expired and should not have remained in storage. The DON stated there was no routine cadence for expired medication disposal and that staff would bring expired medications to her to be wasted with another RN, while facility policy required outdated medications to be removed from stock and destroyed according to procedure, including destruction of discontinued scheduled or legend drugs by two licensed or registered personnel.
