Summary

The facility failed to provide sufficient nursing staff to meet resident needs and to maintain a licensed nurse in charge on each shift. Surveyors reviewed PBJ staffing data, facility assessments, assignment sheets, daily postings, punch details, and staff and resident interviews, and found that the facility repeatedly triggered for excessively low weekend staffing across all three quarters of 2025 and the first quarter of 2026. The facility assessment described a licensed capacity of 127 residents, an average daily census of 108, and a resident population that included short-stay residents, long-stay residents, and an average of 30 to 40 residents with vent/trach and/or dialysis needs. The staffing plan reviewed by surveyors listed multiple RN/LPN and CNA positions across the facility’s three units and two 12-hour shifts, but the daily postings and punch records showed staffing levels that did not align with resident needs. For example, on several reviewed dates in October 2025 and January 2025, the daily postings documented 14 RN/LPNs and 18 or 19 CNAs, with total nursing and CNA hours recorded in the daily postings and different totals reflected in punch details. The assignment sheets showed unit-based staffing patterns for Canyon, Central, and the third unit, with nurses and CNAs assigned on both day and night shifts. Resident interviews described long waits for assistance and delayed care. One resident reported waiting up to an hour for help a couple of times a week, another said he waited 40 to 50 minutes for assistance and remained wet after staff were unable to complete a scheduled change, and another resident reported waiting an hour for help and requesting a bedside commode to avoid soiling herself. A resident also stated that staff were especially short on weekends and that call lights could take at least an hour to be answered. Another resident and her daughter reported that during the dinner period staff all went to the dining room and no one was assigned to the floor to answer call lights. Staff interviews also acknowledged staffing shortages, with a CNA stating the facility often faced shortages and that units helped each other when one unit was short, while an LPN and the staffing coordinator stated staffing was based on census and that each unit typically had one nurse and two CNAs per shift.
