The facility failed to ensure that drugs and biologicals were stored in accordance with accepted professional principles when expired medications were found in a nurse's cart. During an observation on 02/06/2026, surveyors found Linzess capsules for Resident #16 with an expiration date of 12/2025, Clonidine tablets for Resident #32 with a discard-after date of 04/30/2024, and an over-the-counter gas relief product with an expiration date of 12/2025. RN D confirmed the medications were already expired and stated expired medications should not be kept in the cart because staff could mistakenly use them. Resident #16 was a cognitively intact female with rheumatoid arthritis and chronic pain. Her care plan included administering medications as ordered, and her physician order showed Percocet for pain, which could cause constipation. The record review showed she did not have an active order for Linzess on 02/05/2026, yet expired Linzess capsules remained in the cart. Resident #32 was a cognitively intact female with hypertension. Her physician order directed Clonidine 0.1 mg twice daily for essential hypertension, with instructions to hold for blood pressure less than 110/60. Although RN D stated the resident used clonidine in blister packs and not the bottle found in the cart, the expired clonidine tablets were still present. The DON and ADON stated expired medications should not be in carts and that nurses, medication aides, and leadership staff were responsible for checking carts for expired medications.
