The MDS Nurse stated that if a resident who was initially PASRR negative was later diagnosed with a new mental illness such as depression, anxiety, dementia, bipolar disorder, or delusional disorder, a new PASRR I should have been completed. She reviewed both residents’ records and confirmed that no new PASRR I had been done for either resident. The DON stated she was not familiar with PASRR, the ADON said he did not know how PASRR worked, and the Administrator stated there should be a process to determine when a new PASRR was needed after a new mental illness diagnosis.
