Delayed Entry of Verbal NPWT Order and Missing Wound Care Documentation

Summary

The facility failed to ensure that Resident #1’s medical record was complete and accurately documented for wound care after a verbal provider order for a negative pressure wound therapy (NPWT) device was given during wound care rounds. Resident #1 was a cognitively intact male with paraplegia, cancer, aplastic anemia, diabetes, osteomyelitis, and stage IV pressure ulcers of the sacral and coccyx region. The wound care NP was interviewed and stated she gave the verbal order for NPWT during rounds and applied the device to the resident’s sacrum and ischial area, expecting the order to be entered into the EMR and implemented by nursing staff. The provider order for NPWT was not entered into the EMR until several days later, and there was no consistent documentation of care, monitoring, or treatment for the resident’s NPWT device in the EMR during the intervening period. Nursing staff interviewed stated they recalled the resident had the NPWT device and that it was functioning as expected, but they did not document the care in the EMR and did not know why it was not documented. The nursing leadership representative and the Administrator stated the ADON was responsible for entering the order and that the delay in documentation did not meet expectations. Facility leadership stated the ADON had received the verbal order but did not enter it in a timely manner and was later suspended and terminated for the delay in documentation. The facility also reviewed the incident in an ad hoc QAPI meeting, completed a facility-wide documentation audit, and provided in-services on documentation and wound care. The facility policy stated verbal orders must be recorded immediately in the resident’s chart by the person receiving the order and must include the date and time of the order.
