Missed Scheduled Showers for Multiple Residents
Summary
The facility failed to provide necessary bathing and grooming assistance for three residents who were unable to carry out activities of daily living. Record review showed that Resident #1, a male with diagnoses including unspecified dementia, vascular dementia, and hemiplegia/hemiparesis following cerebral infarction, had a BIMS score of 3 out of 15 and required substantial to maximal assistance with showering. His care plan directed showering three times weekly, but the task record showed multiple scheduled bathing dates marked not applicable, including 04/22/2026, 04/27/2026, 05/1/2026, 05/4/2026, 05/6/2026, 05/8/2026, and 05/13/2026. During observation and interview, he was sitting in bed, appeared clean, and stated staff did not shower him but instead he showered himself; he could not recall the last time staff had showered him and said he did like to shower. Resident #2, a male with diagnoses including multiple sclerosis, hemiplegia and hemiparesis following cerebral infarction, and muscle weakness, had a BIMS score of 15 out of 15. His care plan identified an ADL self-care performance deficit and stated he required staff x2 for bathing, but his task record did not show a shower schedule. The task record reflected several bathing dates marked not applicable, including 04/25/2026, 04/28/2026, 05/9/2026, and 05/12/2026. During observation and interview, he was in his wheelchair and appeared disheveled. He stated he thought his shower days were Monday, Wednesday, and Friday, that he was lucky if he got one shower a week, that he never refused a shower, and that missed showers were due to short staffing. Resident #3, a female with diagnoses including restless legs syndrome, muscle wasting and atrophy, and muscle weakness, had a BIMS score of 12 out of 15. Her care plan noted an ADL self-care performance deficit and stated she preferred to bathe independently, while her task record showed scheduled bathing days of Monday, Wednesday, and Friday. The task record reflected multiple dates marked not applicable, including 04/22/2026, 04/27/2026, 05/8/2026, 05/13/2026, and 5/15/2026. During observation and interview, she was sitting in her wheelchair in the hallway and appeared clean. She stated she was supposed to get showers three times weekly, that she received a shower on 05/15/2026 but before then it had been about two weeks since the last shower, and that staff told her they did not have enough help when she did not receive showers. Staff interviews confirmed residents should receive three showers weekly, and multiple staff members stated showers were not being provided as scheduled because of short staffing and that some shower sheets were marked not applicable when residents did not receive showers.
