On 05/19/26, 05/20/26, 05/21/26, and 05/22/26, surveyors observed the Daily Staffing Posting attached to the wall leading to the 200 hall, rather than in a prominent location accessible to all. The posting was located in a common area/dining area immediately after the lobby, with one hall leading to the 100 hall and another leading to the 200 hall, and the posting was positioned on the wall by the 200 hall entrance. During each observation, the posting identified the date, shift times, census of 130, and the number of CNAs, LPNs, and RNs on each shift, but it did not include the name of the facility or the total number of hours worked by direct care staff for each shift. The observations documented the same omission across all four dates reviewed. The Staffing Coordinator stated that the posting should include the type of staff, hours worked, census, date, and the name of the facility, and acknowledged that the current template had omitted the facility name. The facility policy titled 'Posting Direct Care Daily Staffing Numbers,' revised 08/2022, stated that staffing data must be posted daily in a prominent location accessible to residents and visitors and that the form must include the name of the facility and the current date, along with the resident census at the beginning of the shift. The record review showed that the posted information did not fully match the policy requirements because it lacked the facility name and the total number or actual hours worked for each type of direct care staff.

During observations on 6/1/2026, staffing information dated 6/1/2026 was posted at the main entrance, but the census listed as 103 did not match the Resident Listing Report census of 102. Later that same day, staffing information dated 6/2/2026 was observed posted, and on 6/2/2026 staffing information dated 6/3/2026 was posted at the main entrance. On 6/3/2026, staffing information dated 6/4/2026 was posted at the 100 Hall nursing station near the off-hours facility entrance. During interview, the DON confirmed the staffing information posted was dated 6/4/2026 and stated the scheduler would prepare the staffing information sheet before leaving and corrections would be made the next day. The DON also stated the staffing sheet should not be posted in the front and should be placed behind the current day until the new shift starts. Administration stated the staff coordinator should not post the staffing information the day before and should post the current staffing information for the day. The Staffing Coordinator stated she had been putting the staffing information out before leaving at the end of the day for the following day and did not notice the census was incorrect.

During interview, NA-A verified the posting was dated 5/26/26 and stated it was important for families and visitors to see how many staff were working. SC-B stated she changed the nurse staffing hours on Thursdays for the coming week but did not update the posted sheets, and instead made the changes in the computer after they occurred when she returned on Mondays. A review of the next week's schedules showed the census as 24, and one schedule dated 5/30/26 had zeros for all staff listed with a census of 24. The DON verified the nurse staff posting should be posted daily so anyone could see how many residents were in the facility and how many staff were working, and stated it was important so families could have confidence there was enough staff to take care of their family members.
