Unattended Medications Left on Resident Nightstand

Lawrence Street Health Care CenterTomball, Texas Survey Completed on 10-02-2025

Summary

Drugs and biologicals were not stored in locked compartments under proper control when Resident #4’s medications were left unattended on the resident’s nightstand. Resident #4 was a [AGE]-year-old male admitted to the facility with a recent re-admission on 02/01/2025 and diagnoses including dry eye syndrome, restlessness and agitation, sleep disorder, muscle weakness, asthma, chronic congestive heart failure, type 2 diabetes, and dementia. His quarterly MDS dated 09/10/2025 showed a BIMS score of 14 of 15, indicating no impairment. His 09/2025 MAR listed multiple morning medications, including Tylenol, amlodipine, apixaban, aspirin, bumetanide, cyanocobalamin, duloxetine, empagliflozin, ferrous sulfate, folic acid, isosorbide mononitrate, losartan, potassium chloride ER, Toprol XL, azelastine nasal spray, and cyclosporine ophthalmic solution. During observation on 09/30/2025 at 10:58 AM, Resident #4’s nightstand contained a plastic cup with approximately 10 pills, one nasal spray, and one eye drop that were unattended. The resident stated the medications belonged to him and that the medication tech, CMA M, had left them on the nightstand for him to take after he returned to his room; he said they may have been unattended for 10-15 minutes. CMA M stated she placed the medications on the nightstand because housekeeping was cleaning the room and the resident was going into his room, and she acknowledged she knew better than to leave medications unattended. The DON stated medications should be kept with the resident under direct observation during administration and that Resident #4 was not eligible for self-administered medication. The facility policy stated drugs and biologicals must be stored in locked compartments and, during a medication pass, must be under direct observation or locked in the medication storage area/cart.
